The goal of Good¢ents is to help build a stronger community. To meet this end, the Good¢ents Board of Trustees has developed general funding criteria. Funds may not be used for political or religious purposes, payment of utility bills, organized labor activities, fund raising, fund drives, advertising or ongoing operational expenses, including payroll. Please see the Good¢ents FAQs for a complete listing of the criteria. Grant applications may be submitted at any time, however, applications will only be reviewed quarterly. Consult the Good¢ents page for quarterly deadline dates. Stop ... If your organization does not hold a 501(c)(3) status, has received Good¢ents funding within the past 12 months or if your organization received 501(c)(3) status from the IRS less than 1 year ago, your organization is not eligible for funding at this time. If you have questions regarding the Good¢ents Program, call the Good¢ents message center at (907) 452-1151 or (800) 770-4832 or email GVEA Good¢ents. Messages will be returned within 1 business day.Organization Application Packet Checklist Please provide all items listed Application (signed and dated) Cover Letter (with details explaining the project that a potential grant would be used for) A budget or bid for the amount requested, with justifications and itemized details Current organizational budget Most recent IRS Form 990, 990-EZ or 990N Copy of the IRS form affirming 501(c)(3) status (must have been awarded at least 1 year prior to this application’s date) IRS Form W-9 Copy of most recent financial statements List of officers and directors and their addresses General InformationName of Organization(Required) Date Established (mm/dd/yyyy)(Required) Month Day Year Physical Address(Required) Street Address Address Line 2 City State AlabamaAlaskaAmerican SamoaArizonaArkansasCaliforniaColoradoConnecticutDelawareDistrict of ColumbiaFloridaGeorgiaGuamHawaiiIdahoIllinoisIndianaIowaKansasKentuckyLouisianaMaineMarylandMassachusettsMichiganMinnesotaMississippiMissouriMontanaNebraskaNevadaNew HampshireNew JerseyNew MexicoNew YorkNorth CarolinaNorth DakotaNorthern Mariana IslandsOhioOklahomaOregonPennsylvaniaPuerto RicoRhode IslandSouth CarolinaSouth DakotaTennesseeTexasUtahU.S. Virgin IslandsVermontVirginiaWashingtonWest VirginiaWisconsinWyomingArmed Forces AmericasArmed Forces EuropeArmed Forces Pacific ZIP Code Mailing Address(Required) Street Address Address Line 2 City State AlabamaAlaskaAmerican SamoaArizonaArkansasCaliforniaColoradoConnecticutDelawareDistrict of ColumbiaFloridaGeorgiaGuamHawaiiIdahoIllinoisIndianaIowaKansasKentuckyLouisianaMaineMarylandMassachusettsMichiganMinnesotaMississippiMissouriMontanaNebraskaNevadaNew HampshireNew JerseyNew MexicoNew YorkNorth CarolinaNorth DakotaNorthern Mariana IslandsOhioOklahomaOregonPennsylvaniaPuerto RicoRhode IslandSouth CarolinaSouth DakotaTennesseeTexasUtahU.S. Virgin IslandsVermontVirginiaWashingtonWest VirginiaWisconsinWyomingArmed Forces AmericasArmed Forces EuropeArmed Forces Pacific ZIP Code Website Contact Person(Required) Phone(Required)Email(Required) General objectives of the organization(Required)Previous grants received from Golden Valley Electric FoundationClick the + button to add a new rowPurposeDateAmount Add RemoveGrant Amount Requested(Required)Brief description of project or program for which funding is requested(Required)(Provide a brief summary statement from the required cover letter, which describes the project and request.)Other sources of funding to be applied to this projectClick the + button to add a new rowSourceAmount Add RemoveGeneral QuestionsIs this a new project/program within an established organization?(Required) Yes No Is this grant a supplement to an established project/program?(Required) Yes No How many GVEA members will benefit from this project/program(Required)Does the organization have tax-exempt status under section 501(c)(3) of the IRS Code?(Required)(If yes, please provide IRS application for nonprofit status, IRS Form 1023) Yes No Number of full-time equivalent paid employees(Required)Will this grant involve additional employees?(Required) Yes No How many?(Required)Is this organization a United Way Agency?(Required) Yes No Does this organization receive funding from United Way?(Required) Yes No Is this organization affiliated with any religious organizations?(Required) Yes No Have you applied or will you apply for state or federal funds?(Required) Yes No Please explain fully, including amounts, which may be available from those sources(Required)Will this be a continuing project?(Required) Yes No Please explain the source of funds for operation in subsequent years(Required)How do you plan to evaluate the success of your project/program?(Required)(If selected for a grant, a progress report may be required for fund distribution)Any other pertinent information, which would aid in the evaluation of your grant request(Required)Date the funds from this grant, if awarded, would be needed (mm/dd/yyyy)(Required) Month Day Year The following attachments must accompany this application: A cover letter (no longer than 2 pages) describing your project or program for which you are requesting funds.
